Patient Portal

To reduce your wait time in the lobby, please complete the portal set-up and forms.

to set up your patient portal account:

1. Verify your email address.  When you book your appointment, you will receive an email from noreply@ezderm.com to verify your email and set up a username with our patient portal, Sadio.  

2. Create a username & password.  

3. Answer all intake questions on the portal site. 

4. Sign consent form, located under the “Appointments” tab at the top right of the screen.
